First appointment

Important items to bring to your first appointment with Dr Taheri:

A referral letter – A referral letter from your GP or specialist is required to see Dr Taheri to ensure that you are paid your Medicare rebate entitlement. From the date of attendance:

• GP referrals are usually valid for 12 months
• Specialist referrals are valid for 3 months.

X-rays/Scans – Please bring all relevant X-rays or scans with you to your appointment.

• Dr Taheri may send you for further scans or investigations if required.

Here is a helpful checklist of items bring to your appointment:

• Medicare card
• Private health insurance details
• CD, x-rays, scans or films
• Current referral

Review appointment

Please bring the following to your review appointment with Dr Taheri:

• Referral letter – Please ensure your referral letter is valid.
• Images/scans – Please bring all relevant X-rays and scans to your appointment.
• Medicare card
• Private health insurance details

You may be asked to update your:

• Patient registration information – To ensure we have your current contact details
• Medical information – List of medications and known allergies as these may have changed since your last visit.

Fees and billing

Please find Dr Taheri’s consultation fees below.

Please ensure you have a valid referral from your GP or specialist prior to your scheduled appointment to ensure you are eligible for a Medicare Rebate.

Procedures

Dr Taheri is a recognised Surgery GAP provider. Feel free to ask our friendly team about this when you book your appointment.

After your surgery, all follow-up care is included for the first 6 to 12 weeks, depending on the type of procedure. After this period, follow-up appointments are $160 per visit.
